Complex higher-risk and indicated patients (CHIP) percutaneous coronary interventions (PCI) still challenge interventionalists. Coexistence of coronary atherosclerotic ectasia, narrowings and chronic total occlusion (CTO), represents a unique complex scenario where no evidence could support a standard treatment.
